These five steps are required to complete your registration. Pacific Crest will continue to recruit until all spots are completely contracted.
Please read the following documents before proceeding.
Step 1: Complete Member Contract (due upon acceptance)
(You may pay your $600 and “buy-out” your Fuel-a-Thon obligation via this form. See Steps 2 and 3 below)

Step 3: Fuel-a-Thon Submission or Donation (due upon acceptance)

Step 4: Medical Information (due by next camp)
You will receive an email via CampDoc.com to complete your medical information form online.
Step 5: Athletic Physical (due by next camp)
Have your medical provider complete this, and return by the next camp.

Make a list of all the people you know who would be willing to help you pay your fees. Then make copies of this letter and mail a copy to each of them. They complete the bottom portion and return it to us with their sponsorship, which is credited — in full — to your account.
